Community Health Resource Center is a 501(C)(3) organization approved by the IRS effective July 19, 1999.  This organization was established out of a desire to empower the community  through  health education, chronic disease management, parenting classes and services in the community.  Working in homecare within the  community  allowed opportunity to assess the needs and disparities of the elderly, youth and children. While providing case management to high risk infants, pregnant teens, and children with special health care needs, gave more insight into the needs of the families, and the affect a chronic illness  has on the caregivers.  Many disparities have been identified and CHRC has been providing services within the community to "Bridge the Gaps",  to fragmented care.


We believe the children are our future and they have to be nurtured and shaped to be compassionate, productive  citizens.


Seniors are our wisdom from the past, who poured into  us the knowledge to accomplish milestones, sheltered from danger,  and provisions to meet our needs.   We believe it is our duty to provide safe and compassionate care for them as they begin to slow down.
